Accident Investigation Requirements for all injuries and illnesses
Complete the Supervisory Accident Investigation Report within two working days. If you need help with the investigation contact Jerry Cox, (941) 915-4542 or Stefan Genser, (941) 915-3811. Provide a copy of the completed investigation to the Business Center Safety Committee and to Safety and Risk Management (fax 861-5966).
