FMLA for the groups listed below* is administered by Sun Life. To request FMLA contact Sun Life at 1-877- Sun-FMLA (786-3652)
30 days in advance of your anticipated leave, or as soon as possible. Sun Life will provide the appropriate forms to determine eligibility under the Act.
  All BCC employees (bargaining and non-bargaining)
  Clerk of the Circuit Court
  Supervisor of Elections

 

*Sheriff's Office employees contact the Health/Safety Compliance Office at 861-4150 to request FMLA paperwork.

*Employees of the Property Appraiser and Tax Collector contact your agency HR representative to request FMLA paperwork.
