2008 Healthy Living Programs are six to eight week-long programs designed to encourage employees to become involved in their personal health and wellness. A maximum of three programs may be completed for incentives each year. ($25 each)

How do I get started?
A health risk assessment (HRA) must be completed online at Aetna.com through Simple Steps To A Healthier Life®, in order to qualify for the incentives. If you need log-in instructions for Simple Steps click here.   The first time you complete the HRA or update your HRA annually, you will receive a $25 incentive. After completing the HRA, you are eligible to take part in any of the Living Well Incentive Programs (described in detail below) or the Online Healthy Living Programs created by Aetna.  The HRA must be updated annually in order to qualify for incentives.


**Health & Fitness Screening** Know Your Numbers!
Each year the Living Well Program conducts health and fitness screenings throughout Sarasota County. This portion of the Incentive Program earns $25 for the participant. The 2008 Health & Fitness Screening dates will begin Oct 2007 and run through December.  

Instructions for Screening:
Please arrive to your screening having fasted (nothing by mouth except plain water for 8-12 hours prior). Labs will be drawn (finger stick) which include a lipid panel (cholesterol) and glucose (diabetes) in 5 minutes. The fitness screening includes a 3 minute step test, blood pressure, body composition, waist girth and a flexibility assessment. Highly encouraged but optional screenings are the 3 minute step test and flexibility assessment.

Join the MOVE!  Walk your way to better Health with our Pedometer Program affiliated with America On the Move! 


Ready to increase your daily steps?  Eat 100 less calories per day than you are currently eating?   America on the Move is a 6 week walking program designed to achieve or maintain a healthy weight by walking an additional 200 steps per day and decreasing calories by 100 per day.  A pedometer is used to keep track of your steps and motivate you to challenge yourself! 

 As you walk your way better health, you will have access to a number of online tools to help achieve your goals. You have the option to receive daily e-mailed, motivating tips, articles about healthy living, and healthy recipes.  As the steps are entered, an individual can follow his/her progress on trail such as the Appalachian trail, Route 50, or Lewis and Clark’s journey. Members can also see how their friends and colleagues are progressing along the trails.

Smoking CessationTake control of your health and give up smoking forever!  The AMI Stop Smoking Program provides ongoing support to keep you a permanent non-smoker. The support resources provided for AMI clients are designed to help you through difficult times, while also allowing you to give & receive support through the Community Forum. There has been some success with chewing tobacco for this program.

Another option available to employees is to sign-up for the smoking cessation clinic through the Employee Health Center (861-6833). Jane Frary, ARNP will meet with an employee on an individual basis for the first week and then ask that they return two weeks later and then monthly thereafter, for up to 3 months. Jane will write a prescription for Chantix (smoking cessation medication) for all individuals in which it would be medically beneficial to receive. This clinic also qualifies for the incentive program. Please submit the needed forms upon completion of the program.

Gym Membership and/or Fitness & Sports Equipment  

       

Thinking about purchasing new fitness equipment or a membership to your favorite health club?  If so, you are eligible for an incentive, which will help off set the cost!  Equipment such as bikes, kayaks, in-line skates, home exercise equipment, fitness/exercise videos, yoga materials and much more qualify for the incentive.

Get Expert Advice! You can earn an incentive for Personal Training Sessions, Nutritional Counseling Sessions or Weight Loss Programs, such as Weight Watchers (however, these are subject to approval)

On-site Exercise Program Participation

Work Out at Work!  Participate in an on-site, work exercise program or class and earn a $50 incentive while you burn the calories and get healthier!  Examples: Participate in the intern fitness classes, Debbie's Boot Camp, Pilates, Fit Planet Classes or an invigorating exercise program on your own.  To review a complete list of classes or register for a class, please go to TRAC.  If you do not have access to TRAC, email or call  Angela at adeem@scgov.net or 650-9908 or Natalie at nrotella@scgov.net or 650-5294 

Classes can have varying class attendance qualifications, (80% participation is required for most) please ask the instructor for details. After completion of the class, please submit a completed Post Exercise Survey.   This form must be completed in order to qualify for the incentive. 

*Please note that participating in the Pedometer Program cannot be applied to this program congruently but rather sequentially.  Maximum of (3) six or eight week sessions

On-Line Healthy Living Programs

After completing the health risk assessment (HRA), Simple Steps To A Healthier Life®, you are eligible to take part in many of the on-line Healthy Living Programs offered by Aetna. The HRA will recommend specific behavior-change programs for you to consider based upon your results. Each program provides you with a step-by-step approach to making positive changes in your life and in your health. The programs are designed to be fun and informative and can be found under your Personalized Action Plan.  Available programs are:

Healthy Cooking Classes

Frequently we make food choices not in the best interest of our health. Many times we base our decision for what to eat on convenience, familiarity, or availability. Often we aren’t even cognizant of the effects these poor choices may have on our wellbeing. We are left wondering why we have continued weight gain, diminished energy & attention, not necessarily attributing it to what we have eaten. Which is why the Living Well Program would like to offer individuals who have taken a healthy cooking class or disease prevention cooking class to receive an incentive for such initiative. Cholesterol and Triglyceride Management Program

Improve your cholesterol and triglyceride numbers! This individualized program is designed to help reduce your blood cholesterol and triglycerides through adopting healthy lifestyle changes. You will learn the fundamentals of healthy nutrition and how to improve your lifestyle through following a heart healthy nutrition plan, regular exercise and proper prescriptive medication usage if needed (or modification in collaboration with your physician). The program includes at least 4 visits/assessments over a 6 month period with the Jane Frary, ARNP, or one of the RN’s at Employee Health Center. 

If you have known high cholesterol over 200, LDL cholesterol over 130, or triglycerides above 150, this program is for you! Completion of this program meets the criteria for the Living Well Incentive Program. Please call the EHC at 861-6833 to reserve your first appointment.

Blood Pressure Management Program

Get your blood pressure in check! This individualized program is designed to help reduce your blood pressure through adopting healthy lifestyle changes. The program involves following a low sodium nutrition plan, regular exercise and proper prescriptive medication usage if needed or modification in collaboration with your physician. The program includes three visits/assessments with the Jane Frary, ARNP or one of the RN’s at the Employee Health Center. 

If you have borderline high blood pressure (130/80), or high blood pressure (140/90 or higher with or without medication), this program is for you!  Upon completion, this program qualifies for an incentive.  Please call the Employee Health Center at 861-6833 today to reserve your first appointment.

Safety Equipment Purchase or Safety Class attendancesafety
Everyone should make an effort to make their home safer for themselves and their family, now you can earn incentives for doing such.  If you purchase smoke alarms, safety helmets, goggles or some other type of personal or home safety equipment it would qualify for this incentive.  Completion of a safety class could also qualify for this incentive.  Click here for forms and additional information.

2008 Incentive Program Rules & Requirements (Program year begins 11-01-07)

Sarasota County Government continues to support employee health and wellness efforts in many forms. All employees are encouraged to participate in wellness activities in order to maintain or improve their overall health. Full time employees can receive up to $100 for participation in the 2008 Incentive Program.

The on-line Health Risk Assessment (HRA) must be completed or updated between November 1, 2007 and October 31, 2008 in order to be eligible for the 2008 Incentive Program.  
If you have previously completed your HRA in 2007 or earlier, you must update your information in order to receive the incentive. Remember to do this after 11/01/07

  • Log on to www.aetna.com  to complete the HRA, see instructions above.
  • The HRA must be completed in order to participate in all Incentive Programs. (Other Healthy Living Programs are listed above).
  •  If the HRA is not completed, you will be ineligible to receive incentives.
  • Maximum accumulation is $100 per program year. 
  • The 2008 program years begins 11-01-07 and ends 10-31-08. 
  • Incentives are distributed via a lump sum payroll supplement. Payroll distribution dates are as follows (subject to change): 01/31/07; 06/30/08; 11/30/08.
  • Deadlines for completion of various programs, submission of paperwork, HRA up-date or completion is one month prior to payroll distribution dates (or as follows): 12/15/07; 05/31/08; 10/31/08.  For instance, if one updates their HRA 02/20/08 and completes the pedometer program 06/02/08; they would receive $25 in the June payroll distribution for the HRA and then the other $25 incentive would be distributed in November.

Incentive rewards are as follows:

  • HRA completion or update $25
  • Health & Fitness Screening (October 9, 2007 through December 6, 2007)  $25
  • On-line Healthy Living Programs, 6-8 weeks in length, maximum of 3 at $25 each (Simple Steps programs  =  Healthier Diet, Get in Shape, Healthy Heart, etc).
  • Smoking Cessation $25
  • Pedometer Program (America On the Move) $25
  • Gym Membership and/or Purchase of Fitness Equipment $25
  • Nutritional Counseling, Certified Personal Training, Weight Loss Program $25
  • Safety Program or purchase of safety equipment $25
  • On-site Exercise Program participation, maximum of (3)  6-8 week sessions (minimum attendance requirements)  $25 each class series
  • Hypertension Management Program $25
  • Lipid Management Program $25
  • Diabetes classes at Sarasota Memorial Hospital $25
  • Diabetes Management Program $25 

All of the above programs are applied once per program year, except for the on-site exercise program, pedometer program & the on-line Healthy Living Programs (Simple Steps), which can be completed up to three times per year. Please review each individual program instructions and requirements.

  • No partial credit payments will be issued. All required paperwork for each program must be turned in by the program deadlines listed in each specific program rules document in order to receive incentive(s).
  •  Incentives are available to full-time employees, regardless of medical coverage.
  •  Separation of service from SCG forfeits the right to any unpaid incentives.
